This form will capture the student's log-in with an automatic timestamp for that day's distance learning participation. It helps the school with data that can later be used to analyze who and how many students are actually logging in every day. This form is also a tool to encourage the student to practice accessing their SMUSD account on a daily basis whenever possible. To complete this form, please make sure the student logs in with his/her SMUSD student email account and not a personal email account. After signing in to SMUSD, please check the email at http://www.gmail.com for any teacher-to-student communications.
There is no need to call the attendance office or the teacher to retroactively submit this form for a previous day.
